Parent/Guardian Authorization and Form Disclosure
By completing this form, I give my minor child permission to leave the official convocation premises (Atlanta Marriott Perimeter Center) with authorized I.Y.P.U. staff for scheduled trips and outings.
In addition, by completing this form, I agree that Bible Way Church World-Wide, Inc. and the International Young People's Union is released from liability in connection with unforeseen accidents or incidents and subsequent medical treatment pursuant to such accident or incident. Further, the International Young People's Union on behalf of Bible Way Church World-Wide, Inc. has permission to use necessary medical measures in the event of an emergency.
I fully understand the inherent risks involved in the activities my child will be participating in. I accept all risks including those activities preliminary and subsequent to the chosen activities. Further, I give Bible Way Church World-Wide, Inc. and the I.Y.P.U. permission to utilize my minor child's photographs or likeness in promotional materials.
